  1. Review pages 21-47 of the Day 3 course slides and this excel file.
    • Day 3 Slides
    • See the comments for each column heading for descriptions of what information goes into each column
  2. Answer the following questions about control plans
    • What is a control plan?
    • When should a control plan be updated?
  3. Develop a control plan using the attached template, identifying:
    1. What issue needs to be controlled
    2. How the issue will be controlled
    3. What type of control will be used
    4. How often the control will be performed (cycle)
    5. Who will own the control
    6. Status of the control
    7. Any reference materials, documents, SOP’s, etc.

( If you for any reason fail your first attempt,  send me an email and you will be set you up for a re-take at no extra charge.)
The Green Belt is a Proctored Exam and you must show a Government issued ID to the proctor by Webcam.
You must schedule your exam at least 24 hours prior to when you want to take it.  The full instructions for scheduling your exam are on your exam dashboard.
